Navigate Wellbeing Solutions today announced the launch of a new social determinants of health (SDOH) assessment as part of its community support solution. This initiative advances its mission to promote health equity by helping employers, health systems, health plans, and PBMs address social factors such as food insecurity, unsafe living conditions, caregiving challenges, and other wellbeing barriers.
The SDOH assessment enables employees and members to confidentially identify challenges and quickly connect with relevant local support.
Navigate’s wellbeing platform includes access to the findhelp social care network at no additional cost to clients. This network connects individuals to more than 970,000 verified community programs across all 50 states. These programs offer food assistance, housing support, caregiving services, legal assistance, transportation resources, and more available by ZIP code.
Together, the SDOH screening and findhelp social care network provides employees and members with confidential, personalized recommendations in one place, simplifying access to trusted community support.
Health does not begin and end in a doctor’s office. The conditions people live in, such as access to food, safe housing, transportation, or caregiving support, play a powerful role in wellbeing. Our community support solution helps employers, payors, and PBMs address these real-world factors. It ensures people can connect to resources that support healthier lives.
Beyond supporting employees and members, the social determinants of health screening offers aggregated reports for employers and partners, providing insight into unmet workforce needs. These insights help organizations understand the social and economic factors impacting their workforce.
