The inspiration behind our impact

The origin of our innovative model is just as important as its results. Navigate Health Coaching was founded by Dr. Jennifer Musick, PharmD, over 20 years ago. During her pharmacy residency, she witnessed firsthand a critical gap in support for patients with chronic diseases. Many of these individuals needed more help than their primary care providers were able to offer, and that gap became her mission. 

I wanted to make sure that those patients who often get left behind and who need the most help are not forgotten.

After identifying the need for more support in the pharmacy clinic, Dr. Musick was inspired by the success of an initiative serving the employees of Asheville, NC, who were diagnosed with diabetes. The project utilized clinical pharmacists to work closely with these employees and their healthcare providers to better manage their diabetes and improve their health. The results of the Asheville Project were compelling and served as clear proof that pharmacist-managed chronic disease management improved health, reduced risk, and generated a measurable return on investment.  

After reviewing the results of the Asheville Project, Dr. Musick set out to adapt the program and bring it to her community to assist patients with chronic diseases that urgently needed more help.

Condition Management Coaching

With Navigate’s Condition Management Coaching, participants receive care from a clinical pharmacist coach as part of a multidisciplinary team, working together to improve or even resolve high-risk health factors through a personalized care plan. These pharmacists specialize in medication therapy management, helping participants understand what’s working, manage side effects, and build medication routines that fit their daily lives. Combined with whole-person support, this clinical foundation leads to fewer complications, better disease control, and ultimately, lower claims costs and reduced employer plan risk.

Each participant’s coaching journey includes 30-minute bi-weekly or monthly sessions, allowing for more depth and consistency than typical primary care visits. Sessions alternate between a licensed clinical pharmacist coach and a certified health coach, forming a powerful, evidence-based model for sustained change. Pharmacist coaches review lab results, biometrics, and medication histories to identify care gaps and guide treatment adjustments. Certified health coaches then support participants in making those changes stick, using behavior science to address lifestyle factors like stress, sleep, nutrition, activity, and weight.

Lifestyle Coaching

Navigate’s Lifestyle Coaching is designed to help participants make meaningful, lasting health changes with up to five one-on-one virtual sessions with a Nationally Certified Health and Wellness Coach. The journey begins with registration and a Values Assessment, which guides the coaching experience by helping participants identify their core values and motivations. This foundation enables coaches to tailor each session around what matters most to the individual, aligning health goals with personal priorities like nutrition, weight management, physical activity, or stress reduction.

Throughout the coaching program, participants work collaboratively with their coach to establish measurable goals and develop a personalized action plan. Sessions focus on building skills and addressing behaviors that drive sustainable progress, with coaches offering consistent support, accountability, and encouragement. As participants advance in the program, coaches revisit goals, assess obstacles, and adjust strategies as needed. In the final session, the coach helps evaluate the overall experience and guides participants in determining their next steps, whether continuing with additional coaching, reinforcing recent gains, or planning independently for long-term success. 

Weight Management

Navigate’s Weight Management program promotes sustainable lifestyle and behavioral changes to help participants lose weight and keep it off. The program offers personalized support for individuals currently taking or considering GLP-1 medications as part of their weight loss journey. Open to all participants, the program also includes targeted outreach to individuals who meet specialized risk criteria to ensure those most in need of support are engaged early and effectively.

Mental Wellbeing

The Mental Wellbeing program helps participants manage stress and life challenges through structured goal setting and a forward-focused coaching approach. Guided by a professional mental health coach, participants explore personal challenges such as workplace or family-related stress, parenting, grief, coping mechanisms, relationship dynamics, and self-care strategies. Tobacco Cessation

Navigate’s Tobacco Cessation program delivers ongoing, personalized support to help participants quit smoking for good. Wellbeing Results Review

This personalized session empowers participants to take control of their health by interpreting survey (HRA) results, understanding healthy biometric ranges, and identifying recommended next steps to improve their overall wellbeing. Open to all participants, this offering also includes targeted outreach to individuals who meet specified risk criteria, helping drive participation and ensure the right support reaches those who need it most. 

Biometric Screenings

Navigate’s Biometric Screenings provide participants with actionable insights into their health risks using standard and expanded screening panels that include BMI, blood pressure, lipid profiles, blood glucose, and more. The program meets participants where they are by offering onsite, offsite, and at-home options, supported by online scheduling for maximum convenience. Participants also have the option to submit biometric data from their primary care provider using physician forms. Navigate ensures a seamless experience with comprehensive event support, including logistics, communications, real-time dashboards, and direct results submission to participants or their doctors.
